What SERVICES are Offered at Sanity Center?

Individual 1:1 Therapy Session (Children, Adolescents & Adults) - $125.00

EMDR (Eye Movement Desensitization and Reprocessing) Therapy Session - $135.00

Family or Couples Therapy Session - $145.00

Body Image Coaching 1:1 Session - $100.00 in office, additional fees for off-site coaching

We believe that your treatment expenses should not be a mystery and that pricing should be up front, clear, and understandable. One thing that you might notice is that we are significantly more affordable than many valley clinicians. We want to ensure that we are able to offer rates that empower individuals to seek mental health support.

Good Faith Estimate:

Every client will receive a Good Faith Estimate. The Good Faith Estimate shows the costs of items and services that are reasonably expected for your health care needs for an item or service. The estimate is based on information known at the time the estimate was created. The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur. If this happens, federal law allows you to dispute (appeal) the bill.

The Good Faith Estimate is not a contract and therefore does not require you to obtain the items or services provided by Sanity Center, LLC. The foundation of a good therapeutic relationship between a client and therapist is the client’s right to autonomy and self-determination. Therefore, you (as the client) have the right to terminate services at any time.

If you are billed for more than the Good Faith Estimate, you have the right to dispute the bill.

You may contact the health care provider or facility listed to let them know the billed charges are higher than the Good Faith Estimate. You can ask them to update the bill to match the Good Faith Estimate, ask to negotiate the bill, or ask if there is financial assistance available.

You may also start a dispute resolution process with the U.S. Department of Health and Human Services (HHS). If you choose to use the dispute resolution process, you must start the dispute process within 120 calendar days (about 4 months) of the date on the original bill.

There is a $25 fee to use the dispute process. If the agency reviewing your dispute agrees with you, you will have to pay the price on the Good Faith Estimate. If the agency disagrees with you and agrees with the health care provider or facility, you will have to pay the higher amount.

Additional Services and Fees:

1.) Records Requests/Retrieval:

Arizona Statutes allows providers to charge a retrieval/disbursement fee for medical records sent to entities such as law offices, disability claims, and other organizations. Record requests will only be satisfied by what is deemed clinically appropriate as it pertains to the specific nature of the request. While medical records requests will be fulfilled, with the signing of a release form, we ask for at least 7 business days to fulfill the request. Sanity Center charges $.075 per page for medical record requests that are non electronic.

*There will be no fee for records requests/retrieval that fall under the exemptions listed in the Arizona statute as necessary for continuity of care.


2.) Telephonic or Video Check-Ins:

Sanity Center offers telephonic or video (virtual) phone check-ins with active clients. Check-ins are defined as: any scheduled and agreed upon conversation, communication, and/or coordination regarding the client’s care, treatment, needs and/or goals. Sanity Center charges for check-ins due to the block placed on the Therapist’s schedule for this service. This does not include communication regarding scheduling or rescheduling.  Sanity Center charges $35 per 15 minute intervals. 


3.) Court and Legal Fees:

Clients are discouraged from having their providers subpoenaed or having their records provided for the purpose of litigation. Even though you are responsible for the testimony fee, it does not mean that your provider’s testimony will be solely in your favor. Providers can only testify to the facts of the case and to their professional opinion that is within their scope of practice. 

If a provider is to receive a subpoena, the attorney or office staff will need to call Sanity Center and set up a time for the subpoena to be served during office hours. Sanity Center requests a minimum of 72 hours notice of any court appearance so that schedule changes for active clients can be made within a reasonable time frame. Please note: If a subpoena or notice to meet attorney(s) is received without a minimum of 72 hour notice, there will be an additional $250 express charge, which must be paid prior to the appearance in the courtroom. 


When it comes to court action, the following fees are in effect: 

1. Preparation Time (including submission of records): $250/hour (billable in 15 minute increments) 

2. Phone calls: $200/hour (billable in 15 minute increments) 

3. Depositions: $250/hour 

4. Time away from office due to depositions or testimony: $250/hour

5. Written letters or documentation provided: $200/hour (billable in 15 minute increments)

6. All attorney fees and costs that are incurred by the provider as a result of the legal action. 

7. Filing documents with the court: $100 flat fee 

8. The minimum charge for a court appearance: $1500. (A retainer of $1500 is due at least 72 business hours before the scheduled court appearance.)

*The remainder of the costs will be billed after the court appearance and will be due upon receipt. If a provider is subpoenaed, and the case is continued with LESS THAN 48 business hours notice prior to the beginning of the day of the scheduled court appearance and/or the testimony is not given, then the client will be charged $500 (in addition to original retainer of $1500 already paid for having to appear in court). Bills are presented to clients following the court appearance and payment is expected upon receipt.
